Massage Therapy Service Agreement, Informed Consent & Office Policies

Welcome to 81 Massage. Please read the following policies carefully. These policies are designed to provide a safe, professional, and comfortable experience for both clients and therapist.

By scheduling or receiving massage therapy services, you acknowledge that you have read, understood, and agree to the following terms and conditions.


1. PROFESSIONAL MASSAGE SERVICES

Massage therapy is provided for relaxation, stress reduction, pain relief, muscle tension reduction, improved circulation, and general wellness.

Massage therapy is not intended to diagnose, treat, cure, or prevent any disease or medical condition and is not a substitute for medical care, chiropractic care, physical therapy, or treatment by a licensed healthcare provider.

81 Massage is operated by a Licensed Massage Therapist (LMT) licensed through the Texas Department of Licensing and Regulation (TDLR). All services are performed in accordance with Texas laws and professional standards.


2. BUSINESS HOURS & COMMUNICATION

Business hours for responses are Monday through Friday, 9:00 a.m. to 4:00 p.m.

  • Appointment requests are accepted by email or text.
  • Phone calls are not accepted.
  • Messages received outside business hours will be answered during the next business day.
  • Communication should relate only to appointments or massage therapy services.
  • Excessive, inappropriate, or unrelated communication may result in refusal of future services.


3. APPOINTMENTS & BOOKING

New Clients

  • Appointments must be scheduled at least 48 hours in advance.
  • Full payment is required to reserve an appointment.
  • Intake and informed consent forms must be completed at least 24 hours before the appointment.


Returning Clients

  • Same-day or next-day appointments may be available based on scheduling.
  • Clients must notify the therapist of any changes in health, medications, injuries, pregnancy, surgeries, or medical conditions before each appointment.


4. HEALTH DISCLOSURE

Clients agree to provide complete and accurate health information, including:

  • Medical conditions
  • Current medications
  • Injuries
  • Pregnancy
  • Allergies
  • Surgeries
  • Skin conditions
  • Chronic illnesses

Failure to disclose relevant health information may increase the risk of injury or adverse effects.

Clients experiencing fever, flu symptoms, contagious illness, infections, or other communicable conditions must reschedule their appointment.


5. OFFICE POLICIES

Please arrive 5–10 minutes before your appointment.

Late arrivals will receive the remaining scheduled treatment time, and the full session fee will still apply.

Please silence your cell phone before your session.

Children may not be left unattended in the office.

Clients are responsible for their personal belongings. 81 Massage is not responsible for lost, stolen, or damaged property.

Please maintain appropriate personal hygiene before your appointment.


6. SESSION GUIDELINES

Professional draping is required at all times.

Private areas of the body will remain properly covered throughout the session.

Massage therapy provided is therapeutic only.

Pressure, techniques, and treatment areas will be adjusted according to client comfort and clinical judgment.

Clients should immediately communicate any discomfort, pain, numbness, dizziness, or concerns during the session.

Areas excluded during the initial consultation generally will not be added after treatment has begun unless approved by the therapist for safety and scheduling purposes.

Fragrance-free massage lotion or oil will be used unless otherwise requested.


7. PAYMENT POLICY

New Clients

  • Full payment is required at the time of booking.

Returning Clients

  • A $50 deposit is required to reserve an appointment.
  • The remaining balance is due at the time of service.

Accepted payment methods include:

  • Cash
  • Credit or debit card
  • Apple Pay
  • Venmo
  • PayPal
  • Invoice payment

Completed massage sessions are non-refundable.


8. CANCELLATION & NO-SHOW POLICY

A minimum of 24 hours’ notice is required to cancel or reschedule an appointment.

Appointments canceled with less than 24 hours’ notice, missed appointments, or no-shows will forfeit all deposits and prepaid fees.

Exceptions may be considered for medical emergencies or unexpected illness.

If the therapist cancels due to illness or emergency, clients will receive the option to reschedule or receive a full refund.

If a session is terminated because of inappropriate, unsafe, or prohibited behavior by the client, no refund will be issued.


9. PROFESSIONAL BOUNDARIES & SAFETY

81 Massage is a professional therapeutic massage practice.

The following behaviors are strictly prohibited:

  • Sexual comments or requests
  • Sexual behavior
  • Inappropriate touching
  • Harassment
  • Threats
  • Verbal abuse
  • Drug or alcohol intoxication
  • Any behavior that compromises safety or professionalism

Violation of these policies will result in immediate termination of the session without refund and permanent refusal of future services.

Photography, video recording, or audio recording of the therapist is prohibited without written permission.

The therapist reserves the right to refuse, pause, or terminate any session at any time for safety or professional boundary concerns.


10. HYGIENE & RIGHT TO REFUSE SERVICE

For the health, safety, and comfort of everyone, clients are expected to maintain appropriate personal hygiene.

81 Massage reserves the right to refuse or discontinue services if a client arrives with:

  • Poor personal hygiene
  • Strong body odor
  • Strong smoke or cigarette odor
  • Unsanitary conditions
  • Contagious illness
  • Alcohol or drug impairment
  • Any condition considered unsafe or inappropriate for treatment

Appointments terminated for these reasons are non-refundable.


11. LIMITATIONS OF SERVICE

Massage therapy does not include:

  • Medical treatment
  • Nursing care
  • Physical therapy
  • Chiropractic treatment
  • Medication administration
  • Personal care services
  • Bathing or toileting assistance
  • Lifting, transferring, or mobility assistance

Clients requiring these services should arrange for appropriate medical or caregiver assistance.


12. HOSPICE, ELDERLY & SPECIAL NEEDS CLIENTS

Massage sessions for hospice, elderly, disabled, medically fragile, or special-needs clients are intended to provide supportive comfort and relaxation only.

Sessions may be modified, shortened, paused, or discontinued at any time based on client comfort, pain level, fatigue, positioning tolerance, emotional well-being, or safety.

Clients requiring lifting, transfers, mobility assistance, medical supervision, or personal care must have an appropriate caregiver present if necessary.

Reserved appointment time remains subject to the full session fee, even if treatment ends early for safety or comfort reasons.


13. LIABILITY WAIVER

Clients voluntarily choose to receive massage therapy and understand its potential benefits and risks.

To the fullest extent permitted by Texas law, 81 Massage and the therapist shall not be liable for injuries, complications, or losses except in cases of gross negligence or willful misconduct.

Massage therapy outcomes cannot be guaranteed.


14. FORCE MAJEURE

81 Massage is not responsible for appointment delays, interruptions, or cancellations resulting from circumstances beyond reasonable control, including severe weather, power outages, natural disasters, illness, emergencies, or other unforeseen events.


15. EMERGENCIES

If a medical or safety emergency occurs during a session, the therapist may contact:

  • 911
  • Emergency medical personnel
  • Law enforcement
  • The client’s designated emergency contact

The client authorizes appropriate action when urgent medical or safety concerns arise.


16. CONFIDENTIALITY

All client records and personal information are kept confidential and maintained in accordance with applicable laws and professional ethical standards unless disclosure is required by law.


17. POLICY CHANGES

81 Massage reserves the right to modify these policies at any time. The most current version will apply to all future appointments.


18. CLIENT AGREEMENT

By booking or receiving massage therapy services at 81 Massage, clients acknowledge that they have read, understood, and agree to these policies.

Clients understand that services may be refused or terminated without refund due to poor hygiene, contagious illness, intoxication, inappropriate behavior, sexual misconduct, harassment, threats, or any conduct that compromises the safety, comfort, or professionalism of the practice.


The therapist reserves the right to refuse service at any time to maintain a safe, respectful, and professional treatment environment.
